Long term care & Outreach

Our Long-Term Care & Outreach services ensure individuals living in designated long-term care facilities receive personalized, person-directed supports. We focus on dignity, self-determination, and meaningful community and family connections.

Through collaborative planning and coordination, we assist with daily activities, goal achievement, and engagement in community life.

Community outreach

At TCE, our Community Outreach services are designed to support each individual’s unique interests and goals through personalized, community-based experiences. Whether it’s supported work, volunteering, or recreational activities, we empower participants to engage in meaningful, fulfilling experiences. For those who are retired or less physically active, we offer a blend of community and home-based activities, including music therapy, art, cooking, creative expression, and multi-sensory experiences.

Rooted in Person-Centered Planning, our approach places each individual at the heart of all decisions. we tailor supports to personal preferences, interests and hobbies.

The concept of Homeshare happens when an individual lives within a family. Receiving support and sharing in family-life. TCE has one long standing arrangement.
